What is scalp micropigmentation?

The plain-English guide: what SMP is, how it works, what it treats, and what separates work that disappears from work that announces itself.

The Idea

Pointillism, on skin.

Scalp micropigmentation, or SMP, is the practice of placing thousands of tiny pigment impressions into the scalp so they read as hair follicles. Done correctly, the result is a natural five o'clock shadow on a shaved head, or invisible density beneath thinning hair. It is pointillism performed on living skin: each dot means nothing; thirty thousand of them resolve into realism.

Thirty thousand is the working number, and it is often more. A scalp carries somewhere between eighty and a hundred and twenty thousand follicles, but the work is never a one-for-one replacement of all of them. The sides usually keep their own hair and are left alone. What gets built is the density the eye needs to read the pattern as growth, placed only where the pattern has broken down.

It camouflages the visible contrast created by male pattern baldness at every stage, thinning and diffuse loss, women's density loss, alopecia, and transplant or injury scars. It does not regrow hair or change the underlying condition; it removes the contrast that makes the loss read. No surgery and minimal downtime, with short-term aftercare around water, sweat, steam and sun, and occasional refreshes as pigment softens over the years.

How it actually works

A purpose-built machine deposits pigment at a precise, shallow depth: deep enough to stay, shallow enough to hold a crisp point. The result is built over three sessions, letting skin heal and settle between passes so density layers naturally. The result you live with is the healed one, weeks later, not the day-one one. What those weeks look like is in the healing timeline; how it compares to surgery is in SMP vs hair transplant.

Why some SMP looks obvious and some disappears

The difference is rarely effort; it's theory. Skin's own undertone shows through every healed impression, so pigment must be corrected for the person under it; otherwise it drifts grey, blue, or green as it settles. That correction framework, Modern Color Theory, was created by Stevey G and is now taught industry-wide; the drift itself is explained in why SMP turns blue. The other tell is the hairline: drawn too low, too sharp, or too young, it reads as work. Designed for the face and the decade ahead, it reads as you; how a hairline is designed and whether SMP looks real cover both tells in full.

Where to start

Look at healed results (never fresh ones), read how to choose an SMP artist, then talk to the artist. Consultations at our Miami and New York studios are free and private, and you'll leave knowing exactly what your result would look like and what determines the cost.

FAQ

SMP basics, your questions.

Is SMP a tattoo?

Yes. SMP is a tattoo, a highly refined one. Pigment is placed into the skin with a needle and it stays there. What separates it is everything after that: purpose-built machines, SMP-specific pigments and a shallower placement, depositing thousands of follicle-sized impressions rather than lines and shading. Regular tattoo ink and technique on a scalp blurs and discolors, which is why SMP is its own discipline.

How long does SMP last?

It is permanent. Pigment placed in skin stays in skin. What changes over years is depth, not presence: it softens the way hair does, and softening is nothing like disappearing. Most clients never need a touch-up at all. The ones who do usually come back after five years or more, because they want to see it pop again.

Does SMP look fake?

Bad SMP does: too dark, too low, healing blue. Correct SMP is deliberately unremarkable: it reads as a clean buzz cut or added density, and people who don't know simply never notice. The realism comes from color theory and impression control, not luck.

SMP or a hair transplant?

Different tools for different goals. A transplant relocates real hair but requires surgery, cost, downtime, and enough donor hair. SMP is non-surgical, faster, and creates the appearance of density or a full shaved look, and it's also used to conceal transplant scars. Many clients combine both.

What does SMP cost?

Treatments start at $2,500, with all sessions included in that figure. Most complete treatments land between $2,500 and $5,000; the exact number is quoted per scalp because area, density goal and scar involvement all move it. Scar camouflage and correcting another studio's work are scoped separately. See our honest breakdown of what determines pricing in Miami and New York.
